Primary Care

For Appointments:

Osceola Community Health Services Center 

Welcome to your Primary Care Medical Home (PCMH), an enhanced way of managing your health care!  PCMH is not a building, a house, or a clinic. It is a model of care designed to improve the coordination of your health care and your all-around well-being. We are excited to partner with you in this model of care.

We have created Care Teams to take care of your medical needs at all of our Primary Care sites. Our teams consist of: physician, nurse, medical assistant/health support tech, front desk clerk and a referral coordinator. You may also be referred to other health care professionals for specialty care.  As your care team, we will involve you in decisions about your health, and encourage your participation in identifying ways to improve your health. We will try our best to provide you with the physician that you prefer. However, if not available we will provide you with a list of other highly-qualified providers. 

At your request for an appointment, you will be asked by the Call Center to select a physician for your care. That doctor will lead the Care Team you will be assigned to. Each physician has been given a color so your team will have the name of a color, such as red or blue.
