Birth Certificates
For Persons Born in the State of Florida
Apply in person or by mail. The following documents are required:
- Birth Certificate application. Applicants must be eighteen (18) years of age or older.
- One (1) form of photo identification. All identification must be valid (it cannot be expired). For requests sent through the mail, please provide a legible photocopy. Identification may be one of the following:
- Driver’s License
- State Identification Card
- Passport
- Military Identification
- Payment may be cash, money order, credit/debit card, or certified check. Personal checks are not accepted. Do not send cash through the mail.
Additional Information:
- Include a self-addressed, postage paid 9 ½ x 12 ½ envelope with your order.
- A Search Fee of $5.00 applies when the correct name and/or date of birth is unknown.
- $20.00 for the first copy of a birth certificate (includes one protective cover).
- $8.00 for each additional copy purchased on the same date and for the same person.
- $3.00 each for additional Protective Covers.
- $2.50 for shipping fees. This may be paid in lieu of sending a prepaid envelope.

Online Orders:
The Florida Department of Health in Osceola does not accept online orders directly; however, for your convenience, you can process online requests through an independent company that we have partnered with to provide you this service; VitalChek Network, Inc. Death Certificates
For Persons Who Died in the State of Florida – 2009 to Present
Apply in person or by mail. The following documents are required:
- Death Certificate application. Applicants must be eighteen (18) years of age or older.
- One (1) form of photo identification. For requests sent through the mail, please provide a legible photocopy. Identification may be one of the following:
- Driver’s License
- State Identification Card
- Passport
- Military Identification
- Payment may be cash, money order, credit/debit card, or certified check. Personal checks are not accepted. Do not send cash through the mail.

- Include a self-addressed, postage paid 10x15 envelope with your order.
- A Search Fee of $5.00 applies when the correct name and/or date of death is unknown.
- Death Certificates are $10.00 per copy.
- $2.50 for shipping fees. This may be paid in lieu of sending a prepaid envelope.
