Birth Certificates

For Persons Born in the State of Florida
Apply in person or by mail. The following documents are required:

  • Birth Certificate application. Applicants must be eighteen (18) years of age or older.
  • One (1) form of photo identification. All identification must be valid (it cannot be expired).  For requests sent through the mail, please provide a legible photocopy. Identification may be one of the following:  
    • Driver’s License
    • State Identification Card 
    • Passport 
    • Military Identification 
  • Payment may be cash, money order, credit/debit card, or certified check. Personal checks are not accepted. Do not send cash through the mail.

Additional Information:

  • Include a self-addressed, postage paid 9 ½ x 12 ½ envelope with your order.
  • A Search Fee of $5.00 applies when the correct name and/or date of birth is unknown.

The Florida Department of Health in Osceola does not accept online orders directly; however, for your convenience, you can process online requests through an independent company that we have partnered with to provide you this service; VitalChek Network, Inc. Death Certificates

For Persons Who Died in the State of Florida – 2009 to Present
Apply in person or by mail.  The following documents are required:
