Healthy Start provides assistance to pregnant women, interconceptual women, infants, and children up to age three to obtain the health care and social support needed to reduce the risks for poor maternal and child health outcomes.
Healthy Start offers care coordination services to all pregnant women and infants regardless of their socio-economic status to ensure that early care is directed to those families where there is the best chance of preventing or minimizing adverse outcomes.
Healthy Start Offers:
Care Coordination
- Developing a caring and trusting relationship
- Tracking the client’s receipt of services
- Advocating on behalf of the client and family for needed services
- Ongoing systematic assessment of client or family assets, risks, concerns and priorities
- Providing anticipatory guidance and health promotion information, and reinforcing the health care regimen, at each contact
- Empowering the client and family towards self-sufficiency and economic stability
- Promoting employability and life management skills
- Developing and updating a family support plan
- Providing referrals and following up on referrals
- Evaluating the client’s level of care
- Prioritizing, Planning and Evaluating, in conjunction with the family, the actions required to address their concerns, risks and priorities
- Monitoring progress of services
- Transition of ongoing care coordination to other providers, when appropriate
- Maintaining ongoing communication with other providers, especially the prenatal and child health care provider (obtain proper releases for sharing of information if referral to Healthy Start
